- Live Well Suffolk Penalty Shootout Challenge is open to all Suffolk residents aged 18 or over excluding employees of Live Well UK Ltd and Weight Management Centre, their families or anyone else professionally connected with the promotion.
- To enter you must have answered all ten questions of the Penalty Shootout Challenge correctly. Last day for entry is 31st July 2012.
- Only one entry per person is allowed.
- Live Well Suffolk accepts no responsibility for any technical, hardware or software failures of any kind resulting in an entry not being received.
- There is one pair of ITFC game tickets, two sets of four CineWorld tickets and 10 frisbees to be won (certain restrictions apply to the tickets).
- Entrants will be entered into the prize draw which will take place on 3rd August 2012
- The winners will be informed by post or email within 14 days of the draw taking place.
- Entrance into the promotion is deemed acceptance of these terms and conditions.
- Winners may be required to take part in marketing activities relating to the promotion.
- The names and towns of the winners will be made available on or after 31st August 2012 by sending a SAE to Live Well Suffolk, Adelphi House, 8 Turret Lane, Ipswich IP4 1DL.
- The decision of the promoter, Live Well Suffolk, regarding any aspect of the promotion is final and binding and no correspondence will be entered into.
- The prizes are non-transferable and cannot be exchanged for cash.
- Live Well Suffolk reserves the right to hold void, cancel, suspend or amend the promotion without notice, where it becomes necessary to do so.
- Live Well Suffolk reserves the right to offer alternative prizes of the same or higher value.
